Medical Reconciliation Program

This is a direct-access program, sending teams of health advisors into the homes of every member of our community, providing information on a broad range of health-related topics, including: 

  • Information on nutritional issues (organic vs. inorganic, foods with high nutritional values vs. "junk food," foods with high sugar and sodium, etc.).
  • Information on exercise as it relates to health.
  • Training in the correct procedure to follow for checking blood sugar.
  • Reviews of prescriptions and inventories of the medications in the home, discarding meds that have either been replaced or that have expired.
  • Reviews of correct dosages.
  • Information about insurance coverage for family members and assistance with connecting the uninsured to providers, offering information about free and sliding-scale health programs.
  • Medical reconciliation lists for each family member on medications.

This direct-access program will address the health issues that we are seeing in our community that are related to inactivity, poor nutrition, high blood pressure, diabetes, and accidental overdosing with medications.

We will be training 4-6 community members for these health teams. They will receive Community Health Worker Training and will be qualified to integrate the community report back to the health care providers.

This program will raise awareness among our health care providers about the problem we have with patients who continue to take older medications in addition to the replacement prescription. This problem could be resolved if physicians would ask the patient to bring in their old meds at the time they are issuing the new prescription.
